Melissa Lindley from Willamette Vital Health will offer a class for anyone interested in completing Oregon’s Advance Directive or for those who have questions about how to fill out the form. Oregon’s Advance Directive has undergone extensive changes in the last few years, so if you haven’t looked at your existing directive lately, it’s a great time to do so.
Oregon’s Advance Directive is a legal document with instructions for your health care wishes if you are unable to speak for yourself. In your Advance Directive, you may also assign a legal representative to make decisions for you if you cannot make them yourself. During the workshop, Melissa will walk you through the legal form and provide education on the topics addressed in the forms, as well as the Portable Orders for Life Sustaining Treatment (POLST) form, which is another document used to specify a person’s wishes, especially about CPR.
You will receive a copy of Oregon’s Advance Directive, as well as an Advance Care Planning Guide when you attend this workshop.
